Home Care Services
Care at home can take many forms. At MPI Home Care, we have the staff and expertise to care for clients with diverse needs, from simple personal care services to skilled care for complex catastrophic conditions.
Home is truly where the heart is. This is why we work hard to make it possible for the elderly, the ill, or the disabled to live in the comfort of their homes.
A Registered Nurse conducts a free assessment or evaluation of the potential client to determine what home care services would meet his/her needs and designs a plan of care with client’s and/or family’s input and collaboration. Our goal is to help enhance the individual’s quality of life as well as safety and well-being in his/her home.
For a patient in a health care facility anticipating discharge, a Registered Nurse may assess the patient at the facility and make a home visit as necessary to plan and coordinate the patient’s care at discharge and ensure a smooth transition to home.
A Registered Nurse leads the care team and provides supervisory visits to ensure that the plan of care continues to be appropriate for the client.
Care at Home consists of hourly care day or night, from 4 to 24 hours a day to provide services such as but not limited to:
- Personal care
- Medication reminder
- Meal preparation
- Light housekeeping
- Care of house pets
- Take/Accompany client to appointments

When a client’s condition requires the training, education and experience of a nurse, we have Registered Nurses and Licensed Practical Nurses on staff to provide the care. Likewise, we have physical therapists, occupational therapists and speech-language pathologists on staff. Our skilled professionals will work closely with, and under the direction of the client’s physician to develop a plan of care to meet the client’s needs.

MPI’s On-call Registered Nurse is available 24 hours a day, 7 days a week providing direction and guidance to direct care staff whenever necessary. You can sleep soundly a night knowing that a nurse is only a phone call away.
